Screening Services

At C4 Therapy, we believe early identification leads to early healing. Screening is a proactive practice used to detect risk factors, early behavioral patterns, and indicators of behavioral health concerns such as depression, anxiety, suicide risk, trauma responses, substance use, ADHD, and social determinants of health.

Clinical Assessments

Assessment is a structured clinical process used to evaluate a patient’s biological, psychological, social, and spiritual functioning to better understand their behavioral health needs. At C4 Therapy, assessments are conducted after a screening or in conjunction with one, ensuring a complete clinical picture for accurate diagnosis and treatment planning.

For quality assurance, all assessments must be paired with a screening and receive Clinical Director approval before services begin.

Our clinicians provide several specialized assessment types, including:

  • Trauma Assessment – Identifies trauma history, symptoms, triggers, and functional impact to guide trauma-informed treatment.
  • Comprehensive Clinical Assessment (CCA) – A full evaluation of mental health history, presenting concerns, strengths, barriers, and treatment recommendations.
  • Bio-Psycho-Social-Spiritual Assessment – Integrates spiritual and cultural factors with traditional clinical domains to support whole-person care.
  • Psychiatric Assessment for Medication – Conducted by a licensed prescriber to determine whether medication is appropriate and to guide medication management.
  • ASAM Substance Use Assessments & PTSD Assessments – Used to determine level of care and specialized treatment needs.

Diagnosis

Diagnosis is the clinical practice of identifying a patient’s emotional, social-emotional, behavioral, or mental health symptoms and determining whether they meet criteria for a disorder as defined by the Diagnostic and Statistical Manual of Mental Disorders (DSM – current edition) or the International Classification of Diseases (ICD – current edition).

At C4 Therapy, every diagnosis is grounded in a thorough clinical process and must be supported by:

  • A validated screener
  • A comprehensive assessment
  • A clinical conceptualization connecting symptoms, context, and functioning

Our clinicians also consider medical rule-outs, cultural factors, and the influence of prescribed or illicit substance use to ensure diagnostic accuracy and honor individual lived experiences.

Treatment Planning

A treatment plan is a formal, written document that outlines a patient’s current clinical symptoms, diagnosis, and the therapeutic strategies designed to reduce symptoms and support long-term healing. Each plan identifies the patient’s needs, outlines evidence-based interventions, and, when appropriate, includes coordination with affiliated medical or behavioral health providers.

At C4 Therapy, all treatment plans must follow the SMART goal format—Specific, Measurable, Achievable, Relevant, and Time-bound. This standard applies whether clinicians are using:

  • Wiley Treatment Planning tools, or
  • Person-Centered Plans

Every SMART goal must directly align with the patient’s DSM diagnosis, ensuring targeted and symptom-focused treatment.

Therapeutic Services

Therapeutic services at C4 Therapy include a broad range of evidence-based and promising behavioral health practices designed to reduce symptoms, improve functioning, and support long-term health. Services may include psychotherapy, psychiatric medication management, chronic disease support, and substance use treatment.

All onsite service referrals must be staffed with the Clinical Director before or after submission, and final approval occurs during weekly interdisciplinary team meetings.

Care Coordination

Care Coordination at C4 Therapy is a holistic practice that supports patients across clinical, medical, and social domains. It involves navigating and integrating the efforts of primary care providers, specialty healthcare teams, and community resources to promote wellness, improve functioning, and strengthen independence.

Our clinicians help patients access:

  • Healthcare services
  • Housing and utility support
  • Employment and education resources
  • Food and basic needs
  • Assistance with Social Determinants of Health

When appropriate, patients may also be connected to FindHelp.org, 211 United Way, and other community supports.

To ensure ethical and effective communication between providers:

  • All new patients must sign a Consent for Coordination of Care with their primary care provider (PCP).
  • C4 Therapy sends a notification of treatment to the PCP within 90 days of starting services.
  • Patients must renew PCP consent annually.

Care Coordination is built into the therapeutic process to ensure every patient receives integrated, whole-person support.
