Clinical Social Work Services (School-Based)

C4 Therapy (also known as Christ Centered Community Counseling, PLLC) is honored to offer comprehensive clinical social work services designed to enhance the well-being, academic success, and safety of scholars and school communities.

With a licensed team of culturally competent, trauma-informed professionals, we provide both virtual and on-site support to eliminate barriers to care and promote whole-child development. As an NHSC-approved and internationally accredited organization, we are uniquely positioned to support educational institutions with services that are accessible, equitable, and transformational.

Included Core Services

  • Emotional Support: SEL small groups, individual and family counseling, and no-cost mental health services for eligible scholars and families.
  • Resource Assistance: Support with rent and utilities, groceries, beds, clothing, healthcare, employment referrals, and holiday help.
  • Crisis Response: Response to school safety incidents, McKinney-Vento coordination, child protective services involvement, and truancy prevention/interventions.
  • Specialized Consultation: BIP/MTSS behavioral planning, staff wellness and restorative practices, and guidance on state/federal compliance related to child maltreatment, 504/IEP behavioral support, and school policies.

Goals of Clinical Social Work in Schools

  • Improve Scholar Attendance: Implement truancy prevention and chronic absence interventions to increase daily attendance and engagement.
  • Reduce Suspensions & Elevate Academics: Decrease behavioral disruptions and suspensions through Behavior Intervention Plans (BIPs), leading to improved academic performance.
  • Eliminate Barriers to Mental Health Access: Provide culturally competent, licensed therapists on-site, medication management with psychiatrists or nurse practitioners, and day treatment on campus at no cost to families where applicable.
  • Strengthen School Safety & Scholar Wellness: Deliver immediate crisis response, safety planning, and home-visit support to protect scholars and de-escalate high-risk situations.
  • Equip Educators & Staff: Provide staff training and consultation to build capacity for managing emotional trauma, classroom behaviors, and student mental health needs.
  • Enhance Equity & Economic Mobility: Promote equity by connecting families to community resources, hosting monthly engagement events, and supporting long-term economic stability.

Capacity Building Month

  • Contract development, review, and signatures

Month 1: Collaboration & Launch

  • Meet school leadership and educators

Months 2–12: Service Implementation

  • Begin daily services (virtual or on-site)
  • Deliver monthly outcome reports (attendance, behavior, academic trends)

Mid-Year & End-of-Year Reviews

  • Analyze key metrics tied to attendance, discipline, and performance
  • Provide strategic recommendations for sustainability and scale
